Youth Homeless Demonstration Project

Program Overview:

Provides immediate shelter, access to housing programs, guidance, support and resources to youth regarding services available in the community.

In collaboration with Hebron Housing Services and the Youth and Family Project Inc, Family Promise of Ozaukee County provides an assigned System Navigator for wrap-around support to help ensure success in both obtaining and maintaining housing through a plan developed specifically for the individual.

Youth ages 18-24 who are homeless in Ozaukee county are eligible for the program. Youth living on the streets, in their vehicle, in emergency shelters, or fleeing domestic violence will be a priority. Their length of time and number of homeless occurrences are essential factors in determining their vulnerability. Both individuals and families with children are eligible for the program.

The Life Skills Coordinator supports youth with the skills they need to develop according to their housing stability plan in order to maintain permanent housing.
Driven by the “housing first” model, Rapid Re-Housing connects Youth and Young Adults experiencing homelessness to permanent housing through a tailored package of assistance, including time-limited financial assistance and targeted supportive services. Program participants will access case management, housing services, and life skills training.
The Youth Systems Navigator helps program participants obtain and move into permanent housing, stabilize in housing, and connect to services and support as needed. This short-term intervention drives the case manager to focus on helping the youth navigate barriers to tenancy and build a support system by helping them build necessary relationships in their community that support their housing stability plan.
Seven Transitional Housing units will offer limited-term housing and supportive services, including case management, healthcare connection, life skills training, community resource connection, continuing education support, and housing and employment search. Program participants will move into rapid rehousing or permanent housing according to their move-on assessment.

Overcoming Barriers that Contribute to Youth Homelessness

Youth who enroll in the program face the challenge of overcoming significant barriers to housing stability. These barriers include mental and physical health needs, alcohol or drug disorders, limited or no income, little education, and equitable access to services and housing.
